Kilimanjaro Health and Safety: What Every Climber Should Know

Everything you need to know to stay safe and healthy on Kilimanjaro: altitude sickness prevention, fitness preparation, hydration, vaccinations, insurance and what to expect from a well-run climbing crew.

Climbing Kilimanjaro is a physical undertaking at altitude, not a technical mountaineering expedition, and the majority of health risks on the mountain are entirely manageable with the right preparation. The single biggest factor in a safe, successful climb is pace. Guides on every route will set a deliberately slow rhythm — locally known as "pole pole" (slowly, slowly) — because moving too fast is the number one cause of altitude-related problems. Above roughly 3,000 metres the air holds progressively less oxygen, and the body needs time to adjust by producing more red blood cells and adapting breathing and heart rate. Routes that spend more nights on the mountain (7-9 days rather than the bare minimum 5-6) give the body far more time to acclimatise and consistently show higher summit success rates and fewer serious symptoms.

Altitude sickness, medically known as Acute Mountain Sickness (AMS), affects a large proportion of climbers to some degree, typically as headache, nausea, loss of appetite, fatigue or mild dizziness above 3,500 metres. In most cases these symptoms are mild and pass with rest, hydration and a slow pace, but they should never be dismissed. Two more serious conditions — High Altitude Pulmonary Oedema (HAPE, fluid in the lungs) and High Altitude Cerebral Oedema (HACE, fluid on the brain) — are rare but can be life-threatening if ignored, and the only effective treatment is immediate descent. Reputable operators train their guides to recognise the early warning signs and carry pulse oximeters to check blood oxygen saturation and heart rate at every camp, along with portable oxygen and, in some cases, a Gamow bag (a portable hyperbaric chamber) for emergencies. Some climbers choose to discuss the medication acetazolamide (Diamox) with their doctor before travelling, as it can help the body acclimatise faster, though it is not a substitute for a sensible ascent profile.

Beyond altitude, everyday health basics make a big difference on the mountain. Hydration is essential — aim for at least 3 to 4 litres of water a day, since the dry mountain air and physical exertion dehydrate the body faster than climbers expect, and dehydration itself can mimic or worsen AMS symptoms. Drinking water on the mountain is typically drawn from streams or camp supplies and should always be treated (boiled, filtered or purified with tablets) before drinking, as should be arranged by your operator's crew. Sun exposure is another underestimated risk: at altitude UV radiation is significantly stronger, and reflection off rock and, higher up, snow, can cause serious sunburn and eye strain even on cool or overcast days, so high-SPF sunscreen, lip balm, sunglasses with UV protection and a wide-brimmed hat are non-negotiable. Temperatures swing dramatically over the course of a single climb, from warm rainforest at the base to well below freezing at the summit, so layered clothing, a warm sleeping bag rated for sub-zero conditions, and waterproof outer layers are essential rather than optional extras.

Before booking, it's worth having a general medical check-up, particularly if you have a pre-existing heart, lung or blood pressure condition, are pregnant, or are unsure of your fitness level — a doctor familiar with high-altitude travel can advise on suitability and any medication. Travel and medical insurance that specifically covers high-altitude trekking and emergency evacuation (including helicopter rescue where available) is essential and should be arranged before departure; many operators will ask to see proof of this before the climb. On the vaccination side, Tanzania requires proof of yellow fever vaccination only for travellers arriving from, or having transited through, a country with risk of yellow fever transmission, but routine vaccinations (tetanus, typhoid, hepatitis A) and a discussion about malaria prophylaxis for the lower-altitude, pre- or post-climb portion of the trip are worth raising with a travel health clinic six to eight weeks before departure.

Fitness preparation matters more than technical climbing skill — Kilimanjaro requires no ropes or climbing experience, but it does require stamina for consecutive days of 5-8 hours of walking, often on uneven, rocky terrain. Building cardiovascular fitness, leg strength and, ideally, doing several long hikes with a loaded daypack in the months before the trip will make the days on the mountain noticeably more comfortable and reduce fatigue-related risk. Good, worn-in hiking boots are critical, as blisters and foot problems are among the most common (and most preventable) issues climbers face.

Finally, safety on Kilimanjaro is very much a team effort between the climber and the guiding crew. Choosing an operator that follows Kilimanjaro National Park Authority (KINAPA) regulations, employs certified guides with wilderness first aid training, maintains proper crew welfare (fair pay, adequate gear and food for porters, in line with the Kilimanjaro Porters Assistance Project guidelines), and carries appropriate emergency and evacuation equipment is one of the most important safety decisions a climber makes before ever setting foot on the mountain. Daily health checks by the guide team, open communication about how you're feeling, and a willingness to descend if a guide recommends it are what keep summit day both achievable and safe.
